TCM identifies six external pathogenic factors (Liu Yin) that can invade the body and cause disease: Wind, Cold, Heat (Fire), Dampness, Dryness, and Summer Heat. These correspond to climatic conditions found in nature, and TCM considers them pathogenic when they are excessive, when they occur out of season, or when the body's Wei Qi (defensive energy) is too weak to resist them. They typically enter through the skin, mouth, or nose.
Each pathogen has characteristic qualities that produce recognisable symptom patterns. Wind causes sudden onset and rapidly changing symptoms. Cold contracts and slows. Heat agitates and inflames. Dampness is heavy, sticky, and lingering. Dryness depletes fluids. Summer Heat combines heat with dampness. These pathogens can combine — "wind-cold" and "wind-heat" are common combined invasions seen in respiratory illness, while "damp-heat" frequently affects the digestive and urinary systems.
Understanding the specific pathogen involved is essential for correct treatment. A cold caused by wind-cold requires warming, dispersing herbs and acupuncture, while a cold caused by wind-heat requires cooling, dispersing treatment. Prescribing a warming formula for a heat condition — or vice versa — would worsen the patient's condition, which is why precise pattern identification is central to safe, effective TCM practice.

Wind-cold and wind-heat invasions require opposite treatments. Wind-cold (chills, clear runny nose, body aches) needs warming, dispersing herbs. Wind-heat (sore throat, fever, yellow nasal discharge) needs cooling, dispersing herbs. Using the wrong approach worsens symptoms, making accurate differentiation critical.
